The decline of the Islamic civilization was the result of a long process involving many factors both internal and external. However, all of these can be categorized into two sub-categories. Firstly, not understanding Islam and Secondly, not applying Islam correctly.
The lack of understanding of Islam
1 The Neglect in Understanding how Tawheed applies to life’s Affairs, Systems and Relationships (finally resulting in kufr of Secularism and Man-Made Law)
2 General decline in understanding due to absence of Arabic
3 Mujtahideen reduced due to loss of Arabic
4 Orientalist Conspiracy i) Rewriting of Islam ii) Attack on a’qeedah (Qadaa’ wal Qadr) and Ahkaam (Khilaafah, Ta’ddud az Zawjaat, Talaaq, Hijaab, Hudood)

7 Tafarruq (Sects) (Khawaarij, Mutazilah, Raafidah, Qadariyyah, Mujassimah, Naasibah etc.)
8 Secularism (Dunyawi/Deeni split)
9 Foreign Philosophy (Falsafiyah) (Greek, Indian, Roman)
10 Hypothetical Questions (Masaa’il) and Fiqh
11 Hostilities between Madhaahib (Madh-habeeyah)
12 Intolerance of Ikhtilaaf (This is not confined to madh-habs but could be among individuals, rulers, geographical locations etc.)
13 Talfeeq (false fataawa due to jaahil qudaa’ and fuqahaa contradicting the consistent principles of Usoolul Fiqh)
14 Neglect of Walaa’ and Baraa’
15 At Tashbeeh bil Kaafireen (Imitation of the Disbelievers especially the Confusion of Correct principles concerning the adoption of Madaneeyah [Material Progress] and Hadhaarah [Civilization and Culture related to an Ideological Viewpoint])

The lack of implementation of Islam
1 Mismanagement
2 Love of the dunyaa’ causing neglect of duties and progress among umaraa and ummah
3 Non-communication via absence of Arabic
4 Missionary Invasion (Justified by i) Teaching Christians ii) Scientific teaching iii) Humanitarian aid) Consequences among ummah i) Civil unrest and rebellion ii) Love for dunyaa’ iii) Invasion of Western kufr concepts iv) Questioning of A’qeedah and Imaamah v) Divisive concepts such as nationalism vi) Desire for comfort of western material advance vii) strengthening of christians
5 Masonic Conspiracy (merged religions, secular aims, plots against khilaafah, magic and satanic worship)
6 Plots of Munaafiqeen (Always ready to assist enemy)
7 Military Invasion
8 Neglect of Islamic Education (i Fard for all ii Masaajid iii Madrasah iv Jami’yah (universities) v Fard kifaayah [dunyawi] vi Fard Kifaayah deeni [u’loomul hadeeth, u’loomul i’tiqaad, i’lmut Tawheed, u’loomul Quran, i’lm usoolul fiqh, i’lm un Nahw etc.]
9 Neglect of Ijtihaad (Closing the doors of ijtihaad) Relevant to Industrial revolution and new situations and tools of progress and jihaad, comfort and contentment.
10 Mulukiyyah (Hereditary Rule) Neglect of Imaarah and the main functions of khilaafah.
11 Struggle of Ruling Dynasties (Ummawiyyah, A’bassiyyah, Fatimiyyah, Uthmaaniyyah, Saudiyyah etc.)
12 Neglect of Jihaad (Significant in Da’wah and the strength of the kuffaar)
13 Neglect of Amr bil Ma’roof wan Nahi anil Munkar
14 Neglect of Wujoob [Obligations] wal Huqooq [Rights] concerning al Imaarah wat Taa’ah [Authority and Obedience]
15 Mismanagement of Shia’/Sunni split (Doctrinal/Fiqhi/Political division) Sometimes Ahlul Bayt and Shia mistreated and sometimes there corruption was left alone, This could have been treated by education and punishment for both sides.
